VEIT GOALTENDING WAIVER

Veit Goaltending Instruction reserves the right to reschedule or regroup players according to enrolment and availability.  A full credit, minus a $30 administration fee, will be issued upon cancellation, if notified prior to 14 days before commencement of a day or 5 day camp.  Players will be charged the full amount of a private lesson or On-Ice Group Class if notified of cancellation less than 24 hours before commencement of the private lesson or class.

I, the undersigned, agree that Veit Goaltending Instruction, it's instructors, the hockey facility, and it's employees will not be held liable or responsible for any accidents or loss however caused and I agree to release all mentioned parties from all claims and damages.  I further verify that I or my child or my ward have no medical problems and is in good physical health and the above mentioned parties will not be responsible for any medical, dental, or insurances, claims resulting from injury.
